EHS Overview
If an inspector observes a life-threatening exigent health and safety (EHS) deficiency in the course of a REAC inspection, the property will be required to correct or abate the deficiency within 24 hours after the inspection, and certify to the correction or abatement through the online application within three business days of the inspection.

For Public Housing Agencies (PHAs)
These revised instruction explain how the PHA is to certify the correction or abatement of EHS deficiencies.
For Multifamily Housing Property Management
Per HUD, “This form is exclusively for Multifamily Housing property owner use. Property owners should use this form to certify to the correction of the life-threatening (i.e., EHS) deficiencies observed during inspections. Please send or fax a signed copy to your Local Multifamily Housing Field Office until further notice. For additional information regarding EH&S certification, please contact the Technical Assistance Center (TAC) at 1(888) 245-4860.”
Official EHS Form
The EH&S Form is for inspector use only. The inspector is to keep the signed original and provide the property contact with a copy. For a multi-day inspection, the form must be provided at the end of each day if a EHS deficiency is found.
